A desk rejection is an editor’s decision to decline a manuscript without sending it to external reviewers, usually within days of submission. It consumes days rather than months, which makes it the least damaging outcome available and the easiest to learn from — if you can work out which of five things happened.
The letters are short and formulaic, so most of the diagnosis comes from the manuscript rather than the response. A desk rejection has five causes: fit, significance below the journal’s desk threshold, a missing formal requirement, an integrity screen, and an abstract that did not carry the paper. They are not equally common and they are not equally fixable, and everything you do next follows from which one you got.
Fit. The most common by a distance. The editor judged the paper wrong for the readership. Nothing about the work is in question. Fit is decided on shape as well as topic — descriptive versus mechanistic, basic versus clinical, single-centre versus multi-centre, a methods paper versus an application of a method — so a manuscript can sit squarely inside a journal’s stated aims and scope and still be the wrong kind of paper for the people who read it. That mismatch is the one category of desk rejection an author can detect without any inside knowledge, which is the argument for treating journal selection as a decision made before the paper is written rather than after it is finished.
Significance below the desk threshold. High-selectivity journals reject most submissions without review on the editor’s judgement of advance. Not a quality statement so much as a competition statement. The threshold is relative and it moves: the editor is weighing your manuscript against the others that arrived that month and against the number of pages the journal will publish. Journals do not uniformly publish desk-rejection rates, and few publish a desk-rejection rate in a comparable form; many of the percentages in wide circulation come from editing-service posts with no stated method and no denominator, so check the provenance of any specific figure you are quoted. The direction is reliable even where the number is not — the more selective the venue, the larger the share of the decision that is made before any reviewer is invited. A formal requirement. Missing ethics statement, absent trial registration, no data availability statement, word count, reference format, missing author contribution statement. Editors return these rather than fixing them, and the letter often does not itemise which. This is the only one of the five that is usually an invitation rather than a decision, and the only one that is fully determinate before submission: each item is either present or absent, and no editorial judgement is involved in saying so.
An integrity screen. Image duplication checks, plagiarism screens, undisclosed overlap with a preprint or prior submission. These letters are distinguishable: they are specific, and they ask questions rather than declining. A screen that has flagged something produces a query naming a figure panel, a passage or an earlier paper; a rejection on fit never names anything.
The abstract did not carry the paper. The editor read the title, abstract and figures. If the abstract does not state what was found and why it matters within a few sentences, the decision is made on an incomplete picture of a good paper. This is the cause authors most often miss, because nothing in the letter points to it. A letter returning a manuscript on significance and a letter returning a manuscript whose abstract buried its own result are word-for-word identical.
Editorial triage runs as two separate checks with different people, different time budgets and different failure modes, and knowing which one returned your paper tells you how much judgement was involved.
The first is an administrative completeness check, run by editorial staff or by the submission system before an editor sees the manuscript. It asks determinate questions: are the required statements present, is the reporting checklist attached, does the file set match what the instructions to authors demand, is the word count inside the limit. A failure here produces an unsubmission or a return for correction, often within a day, and it is the easiest failure in publishing to prevent — desk screening at scale sets out what is and is not checkable at this stage from a journal’s side.
The second is scientific triage by a handling editor, and it is fast. The editor reads the title, the abstract, the figures, sometimes the cover letter, and rarely the discussion. Fit and significance are both decided here, in minutes, on that reduced object. Author-facing status strings differ between Editorial Manager, ScholarOne Manuscripts and eJournalPress, but the transition that matters is the same in all of them: a manuscript that moves from an editor-held status such as "With Editor" to a reviewer-held status such as "Under Review" has cleared triage. A manuscript that never leaves the editor-held state and then returns a decision was desk rejected, whatever the letter says about “review”.
A third pattern sits between them. Some editors take one informal opinion from an editorial board member before deciding, and the letter may say “after consultation with a member of our editorial board”. That is still a desk rejection: it produced no reviewer report, it is not appealable as a review, and it does not entitle you to a response document, because there is nothing to respond to.
Form letters are chosen from a small dropdown of templates, so the phrasing maps onto the cause more reliably than authors expect. Five phrases carry most of the signal.
“Not a priority for publication” / “does not meet our criteria for further consideration” — significance threshold. Move to a venue with a lower one; the paper is not the problem.
“Better suited to a more specialised journal” — fit, stated plainly and helpfully. Take the advice.
“Please address the following before resubmission” — a formal requirement, and usually an invitation. Fix it and resubmit to the same journal.
Specific questions about figures, data or overlap — an integrity screen. Answer carefully, completely and quickly, and involve your institution if the question is serious.
A generic letter from a journal whose recent contents look nothing like your paper — fit, and one you could have detected before submitting.
Three further phrases are worth naming. “We receive many more submissions than we can accommodate” is the competition statement with no information in it beyond the threshold. “Your cover letter does not indicate why the work is appropriate for the readership of this journal” is fit, and it is the one letter that tells you the letter itself was the weak point — how to write a cover letter covers what belonged in it. The caution that goes with all of this: a template is a template. Do not build a revision strategy on one clause of a sentence an editor selected from a menu in under a minute. Use the phrase to classify the cause, then get the detail from the manuscript.
Formal requirements are the desk-rejection category with published, checkable standards behind it, which means you can check them yourself without guessing what the journal wants.
The ICMJE Recommendations ask that the Methods section include a statement indicating that the research was approved by an independent local, regional or national review body — an ethics committee or institutional review board. Many journals go further and require that body to be named, with its protocol number. A statement that a study was “conducted ethically” without naming the approving committee does not satisfy the requirement, and it is a common version of the failure in submitted human-subjects work.
For trials, the ICMJE makes prospective registration a condition of consideration for publication: the trial must be registered in a WHO ICTRP primary register or on ClinicalTrials.gov at or before the time of first patient consent for enrolment, with the minimum 24-item trial registration data set completed. Two properties of that requirement decide desk rejections. Registration has to be prospective, so registering after enrolment began is a disclosure problem rather than a fix, and the identifier has to be findable — the ICMJE asks that the registration number be published at the end of the abstract, so that is where an editor looks for it. The ICMJE also points authors to the reporting guidelines journals enforce at submission: CONSORT for randomised trials, STROBE for observational studies, PRISMA for systematic reviews and meta-analyses, and STARD for studies of diagnostic accuracy. Journals publishing animal work enforce ARRIVE on the same terms, though the ICMJE does not itself name it. Many journals require the completed checklist as an upload, with page numbers, and return the submission when it is missing — see the CONSORT checklist for the 30 items a trial report must carry, and the STROBE, PRISMA and ARRIVE checklists for observational, systematic-review and animal work respectively.
Data availability is the fastest-growing member of this category. The ICMJE asks that where data have been deposited in a public repository, or are being used in a secondary analysis, the authors state at the end of the abstract the unique, persistent data set identifier, the repository name and the number. In omics work an absent accession — GEO or ArrayExpress, SRA or ENA, dbGaP or EGA, PRIDE for proteomics — is a routine reason a manuscript is returned before review, and “available on request” is increasingly treated as non-compliant rather than as a statement.
An integrity screen is a query, not a decision, and the distinguishing feature is that it names something specific and asks you a question.
The plagiarism arm of the screen is largely automated. Crossref Similarity Check, powered by iThenticate from Turnitin, compares a submission against the full-text corpus that participating publishers contribute, and returns a similarity report to the editor. High overlap is not itself misconduct — methods sections repeat, and the report frequently flags the authors’ own earlier work — which is why the letter arrives as a question. The answer an editor is looking for is a specific account of what overlaps with what, not a denial.
The image arm has moved from human spot-checking to software screening at a growing number of publishers, looking for duplicated panels, reused regions within a figure and splices in gel and blot images. Answer with the original, unprocessed files and the acquisition record, and answer within days rather than weeks.
The third arm is undisclosed overlap. The ICMJE asks authors to give the editor a full statement about all submissions and previous reports that might be regarded as redundant publication of the same or very similar work, and separately to tell the journal when the submitted work has been posted to a preprint server and to supply a link to it. A preprint on bioRxiv, medRxiv, arXiv, SSRN or Research Square is compatible with almost every journal’s policy; an undeclared one is what triggers the query.
Handle all three the same way: answer completely, answer quickly, keep your co-authors informed, and involve your institution’s research integrity office before replying if the question concerns data provenance rather than wording. COPE publishes flowcharts that editors follow in these cases, and a full, prompt answer is what closes the flowchart at its first branch.
The abstract is the object on which fit and significance are actually judged, because it is most of what the triaging editor reads. An abstract that hides the design, buries the effect or states a conclusion the results section does not support converts a strong study into a weak-looking one at the only moment the decision is being made.
The failure is measurable. Pitkin, Branagan and Burmeister (JAMA 1999;281:1110–1111) audited abstracts in six general medical journals, counting an abstract as deficient when it contained data “either inconsistent with corresponding data in the article’s body (including tables and figures) or not found in the body at all”, and found deficient proportions ranging from 18% to 68% across Annals of Internal Medicine, the BMJ, JAMA, the Lancet, the New England Journal of Medicine and CMAJ. Those are the venues with the most editorial resource in medicine, which sets a floor rather than a ceiling for everyone else.
The specific defects that lose a paper its review are narrow enough to check. The design is not named by its accepted label. The primary result appears as an adjective (“significantly improved”) instead of an effect estimate with its precision. The denominators are missing. The conclusion uses a causal verb the design does not license. The identifiers — registration number, accession, funding — are absent. How to write an abstract sets out the six moves an abstract makes and the order in which to draft them; the overclaim check is the pass that catches a conclusion the results cannot carry.
Consider a two-sentence letter received four days after submission to a general medical journal, saying only that the manuscript “does not meet our criteria for further consideration” and wishing the authors well elsewhere. Nothing in it identifies a cause. The diagnosis still runs cleanly.
Four days rules out review. The phrase is the significance template, not the fit template, so the editor did not think the paper belonged in a specialist venue — the fit rejection has its own sentence and this is not it. The submission system’s status never left the editor-held state, confirming no reviewer was invited. That narrows the field to significance or an abstract that failed to convey the advance, and those two are distinguished by re-reading the abstract alone.
Read only the title and abstract: a cluster-randomised trial of a discharge intervention, with the primary outcome stated as “readmission was reduced” and no absolute numbers, no confidence interval, and no statement of the comparator. The design label “cluster-randomised” appears in the methods section of the paper but not in the abstract. The paper is stronger than the abstract, and the editor never saw the difference.
The action follows: rewrite the abstract to name the design, give both arms with denominators and the effect estimate with its interval, then submit to the next venue. Do not appeal, and do not add experiments. The manuscript was never assessed on its evidence.
Diagnose a desk rejection by reading four things in order — the elapsed time, the status history, the letter’s template phrase, and your own abstract — and stop as soon as one of them is decisive.
Elapsed time. Under two weeks with no reviewer reports attached means triage. Time alone does not distinguish the five causes, but it rules out anything you could answer with a revision letter.
Status history. A manuscript that never reached a reviewer-held status was not reviewed, regardless of any language in the letter about the paper having been “assessed”.
The template phrase. Match it against the five mappings above. Fit and significance have distinct sentences at almost every journal, and authors routinely read the first as the second and resubmit sideways into the identical outcome.
Your abstract, read alone. Print the title and abstract with nothing else, hand them to a colleague outside your subfield, and ask what was found, in what system, and why it matters. If they cannot answer in one sentence, you have your cause whatever the letter said.
Two negatives are worth stating. A desk rejection letter that lists no specific defect is not evidence that no specific defect exists — templates carry no detail by design. And a fast rejection is not evidence of contempt: fast is the service, and a journal that takes eleven weeks to send the same letter has done you actual harm.
Check the abstract carries the paper. Read only your title and abstract and ask whether a busy editor would know what was found, in what system, and why it matters. This is the smallest possible change and the most frequently skipped. Rewrite the results sentence so it carries an effect estimate with its precision and both denominators, and rewrite the conclusion so its verb matches the design.
Check the formal requirements against the target journal’s checklist, item by item, before submission rather than after. Ethics statement naming an approving body, registration identifier in a findable place, data availability naming a repository, competing interests, funding, author contributions. Add the reporting checklist for your design with page numbers filled in, and check the instructions to authors for the items the ICMJE baseline does not cover — word limits, reference style, figure formats, structured-abstract headings.
Check fit by reading recent contents, not the aims and scope page. Match on shape — descriptive versus mechanistic, basic versus clinical — as well as topic. A practical version: list the last twenty research articles the journal published, and ask which of them your paper would sit beside without looking out of place. If none, the venue is wrong however well the keywords match. Out-of-scope objections covers the same judgement when a reviewer raises it later.
One thing not to change: the science. A desk rejection carries no assessment of your evidence, so treating it as a signal to add experiments is a serious misreading — that is what rejected after review is for, where you have actual reports to triage.
Resubmitting to the same journal is usually welcome where the rejection was a formal requirement. Fix the missing item, and say in the cover letter exactly what changed. Where it was fit or significance, do not — a desk rejection on those grounds will not reverse, and resubmitting without invitation irritates editors whose goodwill you may want later.
The cover letter for a formal-requirement resubmission is short and mechanical: name each item that was missing, state what has been added, and give its location. “The trial registration identifier (NCT number) now appears in the abstract and in Methods, page 6” is a complete paragraph. Do not re-argue the significance of the work in that letter; nobody questioned it.
Appeals sit outside this. An appeal against a desk rejection succeeds only where the editor made a factual error — mistook the design, missed a section, misread a figure — and not where you disagree with a judgement of priority. When to appeal a rejection sets out the narrow case; the general answer for fit and significance is that the appeal takes two weeks and returns the same decision.
A transfer offer is an invitation to move your manuscript, with its files and sometimes its editorial record, to another journal in the same publisher’s portfolio, and it usually accompanies a fit or significance rejection at a selective venue.
Accepting has two real advantages: the resubmission is mechanical rather than a fresh formatting exercise, and where reports exist they travel, which can shorten the second decision. It has one drawback that authors underestimate: the cover letter travels too. A letter that names its first-choice journal three times arrives, unedited, at the second-choice journal. Rewrite it before accepting the transfer.
Judge the destination on the same criteria you would apply to any venue — recent contents, readership, indexing — rather than on the convenience of the route. A transfer into a journal you would not have chosen independently is a submission you would not have made, and it will produce a paper filed where your field does not read.
A desk rejection is not a record, not a peer-review outcome, and not a statement about the correctness of the work.
There is no shared record of rejections between journals. The next editor sees a first submission, and nothing obliges you to mention the previous one — though where the earlier reviews were substantive and you addressed them, saying so can shorten review at the next venue.
A desk rejection is also not a review, which has three consequences. You are owed no reports and should not ask for them. You have nothing to respond to, so a response document is inappropriate. And the objection that returned the paper has not yet been tested by anyone with subject expertise, which means the significance judgement you disagree with may well be wrong — it simply will not be revisited by that journal.
Finally, a desk rejection is not a verdict on the science and it is not evidence of a flaw. Of the five causes, only the integrity screen is a question about the work itself, and two of the remaining four — fit and significance — are statements about the venue. The five causes here sit inside a wider order of rejection causes, most of which only appear after review.
